HIVCM provides comprehensive case management services to
persons living with HIV. Although our case management services focus on
housing, we also assist with medical, dental, mental health and
substance abuse services and treatment; disability and Medicaid
assessment and applications; food stamp applications, referrals for
furniture and clothing as well as other services and needs identified
by our clients. Our clients are very diverse, some of our clients have
many needs and some simply need a person with whom they feel comfortable talking. Our case
managers establish good relationships with
their clients to ensure the relationship is beneficial to the
client. We hope to assist our clients in becoming independent and
successful.
Assessing Eligibility
Potential clients must be HIV positive, live in the
service area (counties of Richland, Lexington, Fairfield, Kershaw,
Calhoun or Saluda) and cannot be receiving housing case management from
another HIV service provider. If a client has received financial
services from another HIV housing provider, there is generally a 12
month waiting period before a client can transfer agencies.
As our focus is on housing, we seek to assist clients in
obtaining and maintaining housing. Through stable housing, our clients
will be able to better manage medical and other health related
issues, as well as complete or continue their education (if they so
desire) and become employed to ensure a level of independence.
